Det drejer sig is\u00e6r om det fedmefremmende samfund og i nedarvede sociale og biologiske forhold.<\/p><p>Denne m\u00e5de at tale om v\u00e6gt p\u00e5 bidrager til en stigmatisering af overv\u00e6gtige, et forhold, der er dokumenteret helt ned i indskolingsalderen. Derfor er det vigtigt, at vi kvalificerer diskussioner i klassen om, hvordan vi taler og t\u00e6nker om v\u00e6gt, p\u00e5 en \u00e5ben og anerkendende m\u00e5de.\u00a0<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0002f7d elementor-widget elementor-widget-text-editor\" data-id=\"0002f7d\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<h2>Sammenh\u00e6ng mellem v\u00e6gt og d\u00f8delighed<\/h2><p>BMI er et hyppigt brugt udtryk for v\u00e6gtkategori, defineret som v\u00e6gten i kg divideret med h\u00f8jden i meter, opl\u00f8ftet til anden potens. Underv\u00e6gtig er BMI&lt;18.5, normalv\u00e6gtig op til BMI 25, overv\u00e6gtig op til BMI 30 og sv\u00e6rt overv\u00e6gtig opefter.<\/p><p>Den videnskabelige litteratur viser klart, at der er et U-formet forhold mellem BMI og risikoen for henholdsvis sygdom og d\u00f8d (figur 1). P\u00e5 befolkningsniveau er det forbundet med en \u00f8get risiko for b\u00e5de sygdom og d\u00f8d at have et lavt eller h\u00f8jt BMI.<\/p><p>Hvis man unders\u00f8ger den samme sammenh\u00e6ng for andre udtryk for (over)v\u00e6gt, f.eks. kropsv\u00e6gt eller taljem\u00e5l, ser man det samme. Den viser risikoen for at d\u00f8 som funktion af v\u00e6gten, i forhold til BMI 24. Man ser, hvordan d\u00f8deligheden stiger markant b\u00e5de ved lavere og h\u00f8jere BMI v\u00e6rdier. Denne type data skal dog altid tages med det forbehold, at de er observationelle, s\u00e5 de observerede sammenh\u00e6nge kan forklares af andre forhold, der f\u00f8lger med v\u00e6gten. Det fors\u00f8ger man dog at beskytte sig med ved at justere for s\u00e5 mange andre forhold som muligt.<\/i> Figuren er fra (Global BMI Mortality Collaboration et al. 2016).<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-bf08df4 elementor-widget elementor-widget-text-editor\" data-id=\"bf08df4\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>Ydermere ser man ogs\u00e5, at bunden af denne kurve flytter sig mod h\u00f8jere BMI-v\u00e6rdier med alderen (Figur 2). Det fort\u00e6ller alts\u00e5, at meget af den usundhed, vi forbinder med at veje meget, i virkeligheden i nogen grad kan forklares ved andre faktorer end selve overv\u00e6gten (Aune et al. 2016; Global BMI Mortality Collaboration et al. 2016).<\/p><p>Der er lavet en st\u00f8rre meta-analyse, der viser f\u00e6nomenet tydeligere, og hvor BMI optimum ligger over 25 for b\u00e5de m\u00e6nd og kvinder fra 50 og op.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-94c9d65 elementor-widget elementor-widget-text-editor\" data-id=\"94c9d65\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<h2>Sammenh\u00e6ng mellem v\u00e6gt og d\u00f8delighed og godt iltoptag (god form)<\/h2><p>En r\u00e6kke studier viser endda, at det at v\u00e6re i god form &#8211; defineret som havende en h\u00f8j iltoptagelse &#8211; kan beskytte mod de usundhedseffekter, der er er knyttet til (over)v\u00e6gt.<\/p><p>Det skal forst\u00e5s s\u00e5dan, at selv sv\u00e6rt overv\u00e6gtige, der har en h\u00f8j iltoptagelse (typisk defineret som at tilh\u00f8re de bedste 20% af befolkningen), ikke har en h\u00f8jere risiko for at blive syge eller d\u00f8 end slanke, der er i lige s\u00e5 god form og en lavere risiko end slanke, der er i d\u00e5rlig form (Barry et al. 2014; Church et al. 2005).<\/p><p>Der er naturligvis f\u00e6rre overv\u00e6gtige, der er i god form, al den stund at det er sv\u00e6rt at tr\u00e6ne meget, n\u00e5r man er tung, og at tr\u00e6ning i nogen grad beskytter mod overv\u00e6gt, men det \u00e6ndrer ikke p\u00e5, at god fysisk form kan beskytte mod effekterne af overv\u00e6gt.<\/p><p>Man ser det samme, n\u00e5r man justerer for s\u00e5kaldt metabolisk profil, typisk defineret ved blodsukkerkontrol, og m\u00e6ngden af fedt og kolesterol i blodet (Yang et al. 2016; Roberson et al. 2014).<\/p><p>S\u00e5 selv sv\u00e6rt overv\u00e6gtige mennesker, der har blodtryk, blodsukker og blodlipider, der ligger i den gode ende, har en risiko for at blive syg eller d\u00f8, der enten svarer til slankes tal for de samme faktorer, eller kun er ganske svagt for\u00f8get. Disse mark\u00f8rer p\u00e5virkes ogs\u00e5 af, hvor sundt man lever, og i hvor god form man er.<\/p><p>Det understreger endnu en gang, at man godt kan v\u00e6re sund, selvom man er tyk. Ens \u00f8vrige sundhedsadf\u00e6rd kan alts\u00e5 \u201dvinde\u201d over effekten af at veje meget.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ba31bfc elementor-widget elementor-widget-text-editor\" data-id=\"ba31bfc\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<h2>V\u00e6gt som individuelt ansvar<\/h2><p>Det er et gennemg\u00e5ende narrativ i vores kultur, at v\u00e6gten er individets ansvar, p\u00e5 trods af at samfundsm\u00e6ssige, kulturelle og arvelige forhold driver den stigende forekomst af overv\u00e6gt. Det giver naturligvis mening p\u00e5 den m\u00e5de, at samfundet ikke kan g\u00f8re folk slanke igen, men det betyder ogs\u00e5, at overv\u00e6gtige bliver lagt til last for deres v\u00e6gt og stigmatiseret.<\/p><p>Denne stigmatisering sv\u00e6kker den psykologiske trivsel og bidrager ultimativt til at g\u00f8re det sv\u00e6rere for overv\u00e6gtige at v\u00e6lge en anden sundhedsadf\u00e6rd. Frygten for at f\u00f8le sig forkert er derfor en v\u00e6sentlig barriere for mange overv\u00e6gtige i at deltage i idr\u00e6tsaktiviteter.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f440293 elementor-widget elementor-widget-text-editor\" data-id=\"f440293\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<h2>Vi ved grundl\u00e6ggende ikke, hvad der skal til for at tabe sig<\/h2><p>Det ville ikke v\u00e6re s\u00e5 stort et problem at se v\u00e6gt som et individuelt ansvar, hvis vi rent faktisk vidste, hvad man kunne g\u00f8re for at \u00e6ndre det, men det g\u00f8r vi ikke.<\/p><p>Der er lavet flere tusinde studier, hvor man har udsat folk for adf\u00e6rds\u00e6ndringer, der f\u00f8rer til v\u00e6gttab, og man ser helt generelt, at selvom overv\u00e6gtige godt kan tabe sig, s\u00e5 tager de gennemsnitligt set langt det meste p\u00e5 igen inden for et par \u00e5r. Det d\u00e6kker naturligvis over forskellige forl\u00f8b, hvor nogle fors\u00f8gspersoner bliver v\u00e6gtstabile efter fors\u00f8get, nogle forts\u00e6tter med at tabe sig, mens langt de fleste tager enten lidt eller meget p\u00e5 igen. Nogle ender endda med at veje mere end f\u00f8r.<\/p><p>Vi ved meget, meget lidt om, hvad der g\u00f8r, at disse forl\u00f8b er s\u00e5 forskellige, for hvis vi gjorde, ville det v\u00e6re lettere at m\u00e5lrette behandling.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-5ac18e4 elementor-widget elementor-widget-text-editor\" data-id=\"5ac18e4\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<h2>Hvad betyder stigmatisering af overv\u00e6gtige for sundheden?<\/h2><p>Mange overv\u00e6gtige oplever b\u00e5de at v\u00e6re ramt af en direkte sociokulturel stigmatisering og af en frustration over ikke at kunne \u00e6ndre p\u00e5 situationen, hvilket i sv\u00e6r grad kan p\u00e5virke deres livskvalitet og trivsel.<\/p><p>Vi ved fra den videnskabelige litteratur, at den oplevede stigmatisering og tab af kropslig autonomi bidrager voldsomt negativt til personlig trivsel, og til at det bliver endnu sv\u00e6rere at \u00e6ndre adf\u00e6rd (Koponen, Simonsen, and Suominen 2017).<\/p><p>Vi ved ogs\u00e5, at overv\u00e6gtige, der f\u00f8ler sig stigmatiseret p\u00e5 grund af deres v\u00e6gt, er mere tilb\u00f8jelige til at tage p\u00e5, blive syge og have overspisninger, end overv\u00e6gtige, der ikke f\u00f8ler sig stigmatiseret (Schvey et al. 2019).<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-241eaf4 elementor-widget elementor-widget-text-editor\" data-id=\"241eaf4\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<h2>Konklusion<\/h2><p>Det er alts\u00e5 entydigt ikke n\u00f8dvendigvis usundt at v\u00e6re overv\u00e6gtig i sig selv. Det at v\u00e6re i god form kan fuldt ud kompensere for de selvst\u00e6ndige effekter af overv\u00e6gt. Ligesom for slanke, s\u00e5 handler det langt mere om sundhedsadf\u00e6rden. Overv\u00e6gtige rammes blot h\u00e5rdere af s\u00e5kaldt usundhedsadf\u00e6rd.<\/p><p>Derfor er det uhyre vigtigt at fokusere p\u00e5 sundhedsadf\u00e6rden frem for v\u00e6gt eller v\u00e6gttab, s\u00e6rligt for b\u00f8rn og unge.
